Equipment Log Book pages?

It has been a long time .Can I get the pages that were in the a M151 Log book?? :wink:

I see this is a fairly old post, but hopefully somebody will see it. I have a correct empty log book. I have found some forms on this site and also on other sites and have copied them, but does anybody know if somebody has reproduced them on more correct thickness of paper so they would be ready to place into the log book. Mostly interested in the different 2408 versions correct for early 60's. Do any of you remember what we had in the small window on the front of the log book or maybe it varies by unit. On the small window on the back edge we had the the company and the vehicle number in ours it would have been like an A for a company then a vehicle number under it.

My M151 came with the original log book. Inside it contained Forms DA 2408-9, including one filled out at time of sale by the army TO the original private owner stating mileage at time of sale. The plastic window on the log book cover contained a simple index card with handwritten information:
