M151 Logbook Part 3

Other requirements for the logbook include DD Form 1970 - Motor Vehicle Utilization Record (available as a .pdf download) and accident reporting forms. A 2020 version of Standard Form 91 can also be downloaded as a .pdf.

The current version of DD Form 518 can be downloaded as a .pdf, but there's no practical way to print it on a 3" x 5" card. If your printer supports small formats (usually by the rear feed slot) you can use these .jpg images of the 1978 version of DD Form 518 to print both sides on a single card:

If it isn't practical to print by that method, you can open this Word file and accomplish the same thing on 8.5" x 11" card stock. The images are centered, so you can print both sides and use the cut lines to trim the card to size.

It isn't practical to keep loose papers and cards in the log book. One solution is to use a Staples Super Heavy Weight 9" x 12" Job Ticket holder. Cut the top portion off to make a 6.5" x 9" pocket and punch three holes in it. You'll end up with a clear plastic document holder that's a close match to the page size.

Another document that should be in the log book is LO 9-2320-218-12. "Copy of this lubrication order will remain with the vehicle at all times. Instructions contained herein are mandatory."

This Word document has the four pages (two cards - both sides) of the SEP 1982 Lube Order that are sized as log book pages - 6.5" x 9.5". Print on off-white card stock, one page at a time. Faint cut lines are provided.

The original document pages were "plasticized." If you was to duplicate that, I recommend Avery Self-Adhesive Laminating Sheets #73601. Laminate both side of the card before trimming it to size and punching the holes. Be sure to follow the Avery instructions - plastic sheet on the table, sticky side up. Please don't ask how I discovered the importance of that.

when i assembled the logbook for my M151A1, here is the source i used for the forms. getting them reduced to the correct size when needed was done at the local copy store.http://david.brubakers.us/forms/military_forms.htm
